COVID Vaccines - Get Vaccinated

HOW TO GET ON THE LIST:  It is simple.  Call 254-697-7000 and give your name, phone number and email address.  The County has several individuals on standby to take your call.  Milam County will call you when it is your turn to get a shot.  You may also email Amy Guillen at

The following groups are eligible to get on the waiting list and one does not have to be a Milam County resident to get vaccinated:

Group 1A. Health care workers and residents at long term health care facilities;

Group 1B. Those over 65 or those 16-65 with an underlying health condition, including cancer, chronic kidney disease, COPD (chronic obstructive pulmonary disease), Down Syndrom, heart conditions (such as heart failure, coronary artery disease, or cardiomyopathies), organ transplantation, obesity, pregnancy, Sickle Cell disease, and Type 2 diabetes);

Group 1C. Those between the ages of 50-64; and

School and licensed childcare personnel, including those who work in pre-primary, primary, and secondary schools; Head Start and Early Head Start programs (including teachers, staff, and bus drivers); and those who work as or for licensed childcare providers, including center-based and family care providers.

DOWNLOAD AND COMPLETE YOUR CONSENT FORMS:  if you are called to be vaccinated, please download and complete your consent forms prior to arrival at the clinic. This saves time and prevents large gatherings. Below are links to the forms:
